Hart Chart PDF
A Hart Chart is a visual exercise tool made up of letters, numbers, or symbols typically arranged in a grid. In clinical practice, Hart Chart activities may be used to support near-far focusing, saccades, visual scanning, and graded visual-vestibular tolerance. They can be useful when patients report symptoms with reading, screen use, head movement, busy visual environments, or other visually demanding tasks.
Download our Hart Chart PDF for use in the clinic or as part of a clinician-directed home exercise program. This resource includes beginner, intermediate, and advanced chart options, allowing clinicians to choose an appropriate starting level and progress the activity based on the patient’s presentation, symptom response, and treatment goals.
