Step-by-Step Guide: Brand-Daroff Maneuver for BPPV

The Brandt-Daroff Maneuver is a therapeutic exercise designed to reduce symptoms of Benign Paroxysmal Positional Vertigo (BPPV) by promoting habituation and helping the inner ear adapt to positional changes. This maneuver is simple to perform and can be used for long-term symptom management, regardless of which ear is affected.

Starting Position

  • Sit on the side of your bed.
  • You do not need a pillow as long as your head is supported by the surface when lying down.

Steps 1 & 2

  1. Turn your head to the left.
  2. While keeping your head turned, lie down to your right side with your legs on the bed, optionally with knees bent for comfort.
  3. Remain in this position until symptoms subside, plus 30 seconds, up to 2 minutes maximum.

Step 3

  • Drop your legs off the bed and return to a sitting position, putting your head back to midline.
  • Remain in this position until symptoms subside, plus 30 seconds, up to 2 minutes maximum.

Steps 4 & 5

  1. Turn your head to the right.
  2. While keeping your head turned, lie down to your left side with your legs on the bed, optionally with knees bent for comfort.
  3. Remain in this position until symptoms subside, plus 30 seconds, up to 2 minutes maximum.

Step 6

  • Drop your legs off the bed and return to a sitting position, putting your head back to midline.
  • Remain in this position until symptoms subside, plus 30 seconds, up to 2 minutes maximum.
